Harnessing the power of rural communities.

With support from the U.S. Centers for Disease Control and Prevention (CDC) and the Maine Department of Health and Human Services, organizations across Kennebec/Somerset, Aroostook, and Hancock/Washington counties in Maine are working together to better support their community members.


These community partnerships bring health care, public health, and social service groups together to connect people with the services they need, such as stable housing, reliable transportation, and financial assistance, so everyone has a stronger foundation for health and well-being.
